To complete the transfer process, you also need to speak with a member of the MemberRx pharmacy team. It is important that you speak with MemberRx as soon as possible to avoid any disruption in your specialty drug coverage. 

To speak with MemberRx, book your preferred call time.  

IMPORTANT REMINDER: You will need to complete a separate Prescription Transfer form for you and/or each of your family members with a specialty drug prescription(s).
